That error usually means it lost communication with the game, like when it teleports or crashes.

Waypoint files automatically loop back to the start of the file when it gets to the end unless told to do otherwise.

The "return" file is an optional file. If you die, it will use the "return" file to get back to the farming area. You tipically need it if the farming area is far away from the spawn point. If it doesn't exist then it will just go the the nearest waypoint and continue. Most people just plan not to die and don't use return paths.
