MM not seeing character is dead

MM continues to try doing whatever it was trying to do when character died, not realizing he's dead. Not sure how long it goes on for because the MM window is filled with failed attempts to heal or loot. It DOES eventually realize the character is dead and acts accordingly. Has anyone else come across this? It never happens with certain waypoints, but frequently occurs with others.
